When image-obsessed Quentin and Anastasia met two years ago, it was love at first sight.
They knew they wanted to spend the rest of their lives together — and spent thousands of dollars getting plastic surgery to transform themselves into the real-life Barbie and Ken.
The pricey treatments and operations have been funded by their parents. Meanwhile their Barbie and Ken lifestyles are being funded with the proceeds from Quentin’s two successful mobile phone shops.
Now, Quentin and Anastasia are changing their names to ‘Ken’ and ‘Doll.’
“When I laid eyes on Quentin for the first time, I knew that I’d found the love of my life,” said Anastasia.
“We were at a boat party in Saint Tropez and Quentin walked over to me and said ‘I’ve found the Barbie to my Ken.’
“It was so straightforward,” she added. “We both knew we had met a partner who was going to enable us to become the Ken and Barbie we had always dreamed of being.”
“Plastic surgery is our passion,” added Quentin. “We want to spend the rest of our lives together morphing into the dolls we love. Our parents don’t call us Anastasia and Quentin anymore — they call us Barbie and Ken!”
Anastasia and Quentin both confessed their plastic surgery dreams to their parents and they happily obliged their wishes.
Aged just seventeen, Anastasia’s parents paid to for her to have her first surgical procedure to alter her body, undergoing a SGD$7000 nose job.
A year later, her parents splurged on a SGD$11,500 boob job to take Anastasia from an already ample 32C to a 32E.
Meanwhile, Quentin’s parents splashed SGD$7000 on a nose job for their eighteen-year-old son. Then at nineteen-years-old, they treated Quentin to Botox injections all over his face which totalled SGD$1400.
Anastasia, who is an only child, says: “I felt so much better after my first nose job and boob job – it was like having armour!
“My parents were so pleased with my nose job too. I had always had a tiny, ugly bump on my nose but after the surgery, it was slightly less visible.
Quentin adds: “Plastic surgery isn’t ‘normal’ in France. But my mother and father accepted my wishes because they could see that I was intelligent and hardworking.
But despite their new-found happiness from their first cosmetic work, the pair felt there was something missing from their lives.
Anastasia says: “After my nose job and boob job, I was getting so much reassurance in my life from my appearance. But I felt that there was a certain joy missing from my life.
“I wanted someone to push me harder to become the Barbie I wanted to be.”
Anastasia explains: “Plastic surgery is a really taboo thing in France, it’s not accepted here like it is in America for example.
“Quentin and I want to start a plastic surgery revolution! Everyone can be beautiful and everyone can improve themselves with cosmetic work.”
As a rising teenage tennis star, Simona Halep was better known for her cup size than her racket skills.
Her ginormous breasts, though a blessing, was a hindrance to her tennis career.
When she burst onto the scene as a junior champion at the French Open in 2008, Simona’s bountiful chest attracted lots of attention and a growing male fan base.
Her large chest garnered her attention – and won her quite a few male admirers – but for the Romanian tennis hopeful, Simona Halep, it was nothing but a hassle.
She found her 34DD breasts uncomfortable, and it caused her severe back pain and hampered her movement on court, leaving her ranked 500th in the world.
In 2009, at the age of 17, the Romanian opted to undergo a breast reduction, taking her from a 34DD to a 34C.
Before long she rocketed up the world rankings and is now really hitting her stride, climbing to No. 2 in the world for the 2015 season.
“My ability to react quickly was worse and my breasts made me uncomfortable,” Halep said.
“It’s the weight that troubles me. My ability to react quickly, my breasts make me uncomfortable when I play,” Halep said at the time of her operation.
“I didn’t like them in my everyday life, either. I would have gone for surgery even if I hadn’t been a sportswoman.”
Halep’s coach Wim Fissette, a Belgian, who has worked with former world No. 1 Kim Clijsters, said the Romanian was “one of the smartest players” on the tour.
Speaking of Halep’s breast reduction surgery, Fissette said: “I did not know her then, but I have seen some photos.
“She took that decision and it was a good one. I understand that it’s what a lot of people know her for. In the Belgian newspapers it is the same thing.
“But let’s hope she will win a grand slam and then they will start talking about that instead.”

That is exactly what Beverly, has been struggling with, for the past years.
Life hasn’t been a bed of roses for Beverly and it sure doesn’t seem to be relenting. Having been involved in an abusive ex marriage, Beverly suffered more than just emotional trauma.
The physical abuse that was afflicted on her resulted in a nasal disfigurement, something Beverly never did deserve. Her divorce ended the years of suffering she endured, but her disfigured nose was a painful reminder of the tragic and traumatic past.
Wanting to start on a clean slate, Beverly decided to get her nose fixed (rhinoplasty). However, with a tight budget, Beverly settled on Bangkok to have her reconstructive surgery done. Through a friend’s recommendation, Beverly finally decided to get her surgery done.

Upon reaching the clinic, Beverly felt that the clinic wasn’t quite the plastic surgery clinic she expected. Though decent looking, it didn’t have the vibes of a proper plastic surgery clinic. However, since a hefty amount was already paid, Beverly dismissed the red flag and waited for her turn.
According to the distressed mum, “I think the doctor was also not experienced because when I was on local anesthesia, there was another lady lying beside me. At first I was confused, but in the end, I just carried on because I really wanted to get the surgery done quickly and I have already paid the deposit.”

Just when things were starting to have a rosier outlook after the surgery, the post surgery outcome started to go awry 8 months after her nose surgery.
What initially looked like a tiny ‘pimple’ growing on the tip of her nose soon overtook her entire nose tip.
“At first I thought it was a pimple, but slowly, my skin started to peel. When I could see my implant jutting out of my nose, I knew something was very wrong. The pain started to come and what’s worse was that it affected my breathing.”
It didn’t take long for Beverly to learn that this growth was actually a botched nose job. In just two weeks, Beverly’s nose started to peel off, leaving only a thin layer of cartilage with the partially exposed silicone implant.
“I was so shocked but I felt more lost because I wasn’t expecting this at all. I didn’t know what to do or who could help me since I did my surgery in Bangkok.”
Anxious but heavily pregnant, there was no way Beverly could take a flight out to consult her plastic surgeon in Bangkok. Desperate, Beverly immediately sought help with the surgeons in Singapore who then told her that the botched job had caused the implant to force its way out, leaving a mess of complication and infection.
With no other solution, her surgeon told her to remove her implant immediately before the infection spreads to the other parts of her nose.
After the removal of the nose implant, this is how Beverly’s nose now looks like.
But the removal of the implant wasn’t an easy option for Beverly since she was heavily pregnant and the surgery required her to go under general anesthesia.
Caught between a rock and a hard place, Beverly had to opt for the removal of the implant to prevent further disfigurement to her nose and very possibly, her face.
Beverly now regrets having done her procedure done in Bangkok which caused her nose to be in this current ‘state’.
However, Beverly’s decision to go under general anesthesia came at a hefty price as her baby was born prematurely at 30 weeks. Weighing a mere 970g, her baby had to be hospitalized for 2 months, resulting in hospital bills to chalk up to SGD$65,000.
The costs of the implant removal and the hospitalization fees have decimated her savings, but her ordeal is far from over. Beverly now faces permanent disfigurement should she not have reconstructive surgery done soon.
Her nose tip is shrinking and becoming shorter by the day and she faces a very real possibility of an inverted nostril with each day she doesn’t have her reconstructive surgery done.
It is more than just her savings being wiped out, but the ordeal has completely washed out her self-esteem and confidence.
Before and after the botched plastic surgery
Having to hear her child comparing her to that of a witch or being unwilling to entertain the thought of wedding bells at the end of the year with her current fiancé due to her disfigurement, pains Beverly.
She has now become a recluse who seeks comfort in her isolated world of her room. The former beautician has her confidence in shambles and is in need of help to piece her sense of self-worth back together again.
“I used to love taking photos with friends and family in the past, but now, I don’t even want to look in the mirror because it is just so terrible. I want to be confident again but I don’t know how to, with a nose like that. Right now, I really need my job back as a beauty adviser because I need the money to bring up my baby, but who would want to hire me?”
Beverly didn’t have it easy with her plastic surgery journey. If there’s one thing we can learn from Beverly’s experience, it would be to always get an experienced surgeon no matter how small the surgery may be because you will end up paying way less and achieve that aesthetically pleasing and desirable outcome you’ve always dreamt of.
Although lipoplasty is currently the plastic surgical procedure performed most often in the United States, the fundamental technique and technology changed only slightly during the past 30 years. Performance of superficial lipoplasty expanded the boundaries of body contouring by enabling the removal of fat from the superficial layers. However, this advance in lipoplasty technique also involved the increased risk of scarring, waviness, and contour irregularities, when excessive fat removal was performed. These issues led to various recommendations to avoid over-resection.
Limitations of Traditional Liposuction
Some of the earliest reports on ultrasound-assisted lipoplasty (UAL) was published in the literature in early 2000. The UAL device broke down fat cells to produce an emulsion, leading to a less traumatic procedure. However, the technological limitations of UAL resulted in a high incidence of complications,, such as burns, skin necrosis, and scarring.
The first reports on the clinical application of a third-generation ultrasound lipoplasty device that utilized pulsed low-power ultrasound and high-efficiency, small-diameter solid titanium probes was VASER (vibration amplification of sound energy at resonance) lipoplasty. The energy applied to the tissues was approximately one-quarter that of previous devices, while the pulsed mode reduced heat generation. Expanded applica-tions of VASER lipoplasty include treatment of the male and female breast, face and neck; fibrous body areas (trunk and back), and combined excisional body contouring procedures of all types Through these applications, VASER was confirmed as an enabling technology to address the superficial fat layer.
Traditional lipoplasty techniques often fail to achieve the aesthetic goal of a “washboard” abdominal contour because “subdermal fat obscures the muscular detail.”
In 2003, Hoyos presented a significant improvement in technique at a Colombian National Congress, which embodied a new approach to body contouring that he termed “high definition liposculpture” (HDL). The term “liposculpture” defined the technique as not simply fat removal but as an artistic approach designed to emulate surface anatomy. HDL was developed through the study of art and anatomy of the human musculature, as an artistic treatment of the human form to create not only a slim figure, but also the appearance of a highly developed musculature.
High-definition liposculpture elevates the concept of abdominal etching to a 3-dimensional approach, taking into account the contributions made by other muscle groups to integrate the entire torso, legs and back. The differing aesthetic goals of male and female body contouring are integrated into this method, as are key areas such as the pectorals in men and the gluteal area in women. However, the potential benefits of HDL alone could be achieved only by highly-trained surgeons, while the technique itself was exhausting for the surgeon. Adverse consequences of the procedure included a very slow healing time and prolonged swelling, which in turn required close, long-term follow up by the surgeon to avoid complications and undesirable results.
Why VAHDL Gives the Most Desired Results of All
The combination of VASER and HDL represents a marriage between technology and technique that allow a more precise, less traumatic procedure and improved outcomes. VASER -assisted HDL (VAHDL) allows the fat to be prepared like an artist’s clay through emulsification, so that the surgeon can “sculpt” muscular anatomy in great detail through gentle extraction aspiration that minimizes the trauma normally associated with traditional lipoplasty.
VAHDL embodies the ultimate understanding of how superficial anatomy influences external appearance. Developed through the study of “surface anatomy” of human musculature much as an artist would view the human form,28-30 VAHDL begins where superficial lipoplasty ends. VAHDL highlights the importance of contributions made to the aesthetics of the human form by both the superficial and deep fat layers when these layers are properly proportioned both between and over the mus- cle groups. What makes VAHDL different is that we use the underlying structures as a guide to contrast the various structures that make up the 3-dimensional body contour.
The Liposuction Situation in Singapore
Unfortunately, the Vaser machine was sold to general practioners (GPs) who wanted to hone their skills in body contouring. These GPs who had minimal or no previous surgical experience or qualifications were given the opportunity to perform minimally invasive procedures that could have devastating effects if performed wrongly.
The machines were sold in quick succession to GPs and they attended a crash course of 3-5 days and experimented on human subjects who were found by the GPs. After completion of the course, they were then licensed to perform liposuction on the general public in Singapore. The GPs started to aggressively market the procedure and were aggressive in the pricing of such procedures. There was also no regulation on the use of deep sedative agents in the clinics where such procedures were carried out.
This alarmed the plastic surgical fraternity who called for more safe practices in a procedure that could even go wrong in the hands of plastic surgeons. Finally there was a case of death caused by liposuction and it was indeed performed by a GP. The case received huge coverage and the death was caused by both an overdose of sedative agents and puncture of vital organs.
There was a call for more safe practices and the Ministry of Health set up a liposuction committee. They could however not reverse their decision as they had previously allowed such a practice.
The remaining GPs who had been performing the procedures slowly gave up the practice safe for a few members. The regulation continued to grow tighter by the year to force more GPs to give up their practice of liposuction.
At present, liposuction in Singapore is mainly performed by plastic surgeons. There is however a difference. Most surgeons who perform liposuction perform it as a debulking procedure. Having performed liposuction for more than 10 years now, I can safely say that the practice of liposuction has undergone a huge transformation. The process of debulking has occasionally created results that do not quite appeal to the patients. The evolution of VAHDL has made more inroads in achieving the beautiful contour that most patients desire. It is able to give patients the results that they expect and it is able to create a more healthy and athletic build that they desire. This in turn changes the patients’ psychology and motivates them to exercise consistently to maintain their results.
Dr Shenthilkumar’s Professional Take on Liposuction
“I have personally been performing these procedures using suction assisted vibroliposuction. That procedure gives quite a lot of swelling, bruising and downtime for patients. The advent of Vaser changed it and gave better results. Even though I had attended training in Italy for this procedure and found that my results had improved a bit, I was still not completely satisfied with my technique. The patients however did not complain as the debulking and contouring still achieved most of their expectations.
After having trained in the art of VAHDL, I can safely say that the procedure is now very predictable and gives results that I thought were not even possible. I must say that I would personally go for such a procedure for myself to be able to achieve such results, although my BMI is at 20. The results themselves motivate one to lead a more active lifestyle.”

Fuller breasts, breasts with better shape, size and balance, restoration of breasts after giving birth – there are many reasons why women want breast augmentation surgery, but the objectives are always similar – to enjoy their desired self-image, and to be more self-assured in their daily lives.
With advancements in surgical techniques, most breast augmentation can be completed in under two hours and post-operative recovery takes only about two to three days. The techniques have also become much more accurate in achieving the desired profile.
He reveals that’s in Singapore the most common methods are implants and fat transfer. For implant augmentation, saline and silicone implants are usually used, with silicone preferred, as it feels more natural. A volunteer firefighter from Mississippi whose face was burned off during a home fire rescue received the world’s most extensive face transplant, New York University Langone Medical Center said on Monday.
After a 26-hour surgery performed at the New York hospital in August, 41-year-old Patrick Hardison is living with the face of 26-year-old David Rodebaugh, a BMX extreme bicycling enthusiast from Brooklyn who was pronounced brain dead after an extreme cycling accident.
Patrick Hardison was working as a volunteer firefighter in his hometown of Senatobia, 40 miles south of Memphis, when he got a desperate call. A house was in flames, with a woman trapped inside. Hardison arrived on the scene and raced inside, moments before the roof suddenly collapsed. His helmet was knocked off, and he felt his mask melting. He closed his eyes and jumped out the window.
Hardison lost his eyelids, ears, lips and most of his nose, as well as his hair, because of that fire. He also had disfiguring third-degree burns across his entire face, head, neck and upper torso. His skin was so badly damaged that he was not even able to close his eyes completely.
“From that day on, Sept. 5, 2001, there was no normal tissue left throughout his face,” Eduardo D. Rodriguez, chair of plastic surgery at NYU Langone Medical Center, said in recounting the first responder’s story.
In the most extensive plastic surgery face transplant surgery to date, Patrick Hardison received a full scalp and face, including ears, nose, lips and upper and lower eyelids.
For the first time since that raging fire in Senatobia, Mississippi in 2001, Hardison can blink and even sleep with his eyes closed – key steps to sparing his blue eyes from blindness that previously seemed all but inevitable, said Dr. Eduardo Rodriguez, the plastic surgeon who led the 150-person medical team which comprised of doctors, nurses and technical assistants that performed the procedure.
Simultaneous surgeries took place, [Dr. Eduardo] Rodriguez said, with Hardison on one operating table while Rodebaugh was on the other. The NYU medical team had practiced for a full year to get it right.
“You only have one chance to land the Rover. The same goes with the face,” Rodriguez told Reuters following a news conference in New York earlier on Monday.
The team slit the skin at the back of the donor’s head, peeling each side forward with key pieces of bone attached at the chin, nose and cheekbone and then precisely draped it, like Batman’s cowl, onto Hardison’s head.
“Everything has to be perfectly positioned,” Rodriguez told Reuters, including the bones, muscles, ear canals, lips and nerves.
The entire procedure cost US$1million, and will be paid for by a grant from NYU.
Proof of the surgery’s success was obvious after a medical team took Hardison shopping for new clothes at Macy’s this fall, and no one in the store gave him a second look, Rodriguez said.
Rodebaugh’s mother, who gave permission for the transplant, noting her son was an unexpected gift after she had been told she could not conceive a child, recently was shown a photograph of the surgical results.
“Patrick is beautiful,” she told the medical team.
“I am deeply grateful to my donor and his family,” Hardison said in a statement. “Even though I did not know who they would be, I prayed for them every day, knowing the difficult decision they would have to make in order to help me. I hope they see in me the goodness of their decision.”

1) What are some of the key areas on the nose that patients are generally unsatisfied with?
Common problems include a low nasal bridge, bony hump, crooked, broad and drooping nasal tip, widened alar base and retracted columella. However, these common problems can be resolved with a Rhinoplasty or commonly referred to as a “nose job”.
2) What does a nose job do for the overall facial appearance?
A good nose job gives the face a slimmer, sharper and more three-dimensional appearance. Also, the angle between the columella and the lip would be greatly improved from the side profile. But more than just the front and side profile, for patient with a receded upper lip (which makes them appear older than they truly are), a Rhinoplasty well done can make them look much younger.
3) What are the different methods adopted for a nose job?
There are both minimally invasive and surgical procedures in the market now.
Minimally invasive procedures such as hyaluronic acid filler injection as well as thread insertion are unable to produce the result of an ideal nose. Besides, the Thread Insertion method is relatively new, so the long term risks of scarring and thread extrusion is still unknown
Surgical techniques include closed and open Rhinoplasty which are more versatile and can produce better results.
4) Does the L shaped implant produce the best results?
Although the L shaped implant has the ability to improve the dorsal height and tip projection. It has limited ability to augment a contracted columella or narrow the nose tip and alar base.
L-shaped implant is not commonly used during my advanced training in Korea as the use of L shaped implant for a Rhinoplasty procedure can cause frequent implant extrusion and deviation.
5) What is the difference between an open and closed Rhinoplasty?
A closed rhinoplasty involves only a small incision on the inside of the nose. However, Its effectiveness is limited compared to open rhinoplasty which visualises all the structures in the nose for definitive correction.
The typical Korean-style open rhinoplasty for an Asian nose involves a small cut across the columella, placement of silicone implant, cartilage from the nose septum and ear cartilage to achieve a natural-looking and refined nose profile. However, there are instances where some patients do not have sufficient cartilage of their own, if so, rib cartilage may be used.
6) What can fillers do for the nose and does it deliver promising results?
There is only so much filler injection can do for the nose. Although filler injection can improve the height of the nose bridge, it is not able to narrow a broad nose tip and wide alar base. While it provides immediate result, it is only temporary and lasts for about six months. There are also infrequent complications such as alar skin tissue loss and blindness.
“I have encountered several patients who had received repeated filler injections by their previous doctors, resulting in a broad and undefined nose bridge due to migration of filler. Filler in this region may take longer to resolve compared to injection elsewhere in the face.”
7) How long is the recovery for Rhinoplasty and tips for recovery?
Most patients are able to return to work in one to two weeks after the procedure. The swelling subsides after a month and the final result will be assessed in about three to six months
After surgery the nostrils are sponge dressed to prevent bleeding for 24 to 48 hours. Patient should also wear a supportive splint over the nose for one to two weeks and sutures are removed one week.

1) What is the difference between the suture vs partial incisional vs incisional method? And who is suitable for which procedure?
Suture methods have tiny incisions and hence less scarring. Patients with less fatty eyelids and who want lower folds are more suitable.
Incisional methods are typically more definitive. Through the longer incision, fat can be removed, muscles can be tightened to enlarge the eye aperture and anchorage of the double eyelid is more secure throughout the length. Although the scar is longer, if performed well it heals to look like a natural double eyelid skin crease.
Partial incisional methods shorten the incision and hence the scar. As the access is shorter, control and anchorage of the double eyelid crease at the inner and outer aspects is compromised. This may result in shallow folds at the inner and outer aspects. As my incisional scars heal well, there is little reason for me to perform this technique.
2) What are the pros and cons for incisional and non-incisional?
Non-incisional: Less downtime primarily due to not removing the fat (Fat removal is the main cause of swelling). Potentially less durable especially if performed in patients who have heavier or fatter eyelids and there is less initial scarring.
Incisional: It is more reliable and durable. Skin and Fat removal or sculpting possible for this incisional method. There will be more swelling due to fat removal and visible scar in the initial recovery stages. However, scarring is not an issue if surgery is performed well as it looks like a natural double eyelid crease.
3) What is done for each type of method?
Non-incisional: Desired height is marked. Usually performed under local anaesthesia and light sedation. I perform the Double Strand Twisting (DST) method. 3 small puncture sites in the upper eyelid through which a fine stitch is passed to anchor the skin to the underlying cartilage.
Incisional: Desired height and skin removal marked. Usually performed under local anaesthesia and light sedation. Excess fat which prevents anchoring of the double eyelid is then removed. Muscle tightening performed if necessary. Skin anchored to underlying muscle and cartilage.
4) What determines a good upper blepharoplasty?
Symmetrical results with non-visible scars, even with eyes closed. Height should be harmonious with the patient’s face, gender and race characteristics. Shape should be aesthetically pleasing with a mild lateral (outer) flare. Tapered or parallel according to patient’s desires.
5)What can upper blepharoplasty do for the eyes?
An upper blepharoplasty can make the eyes look bigger and brighter. Ptosis correction can be performed concurrently. Excess skin causing hooding can be removed. Uneven or asymmetric eyelids can be made more symmetric. Partners bring their boyfriends / husbands because they complain that their single eyelids make them look too fierce.
6) Scarring for both procedures? Are there any ways to remove the scars?
The scars of my upper eyelid patients have all healed quite well, there has been no request to improve the scarring. For some patients operated elsewhere, I have performed lasers to improve the scars.
7) Lifestyle changes before surgery?
Smoking cessation only, it slows down healing.
8) Foods to avoid or things to do before surgery?
No foods to avoid. Stop smoking 2 weeks before. There is a list of medications and supplements to avoid from 1-2 weeks before surgery. As we usually sedate our patients, we ask them to fast 6 hours beforehand.

Qn: Is the fat transfer method effective?
Ans: Each session of fat transfer to the breast can increase the breast volume by half to one cup size. This can result in soft and natural breast consistency.
In the fat transfer procedure, fats is harvested from the abdomen and thighs in a non-traumatic fashion. It is then processed after centrifugation.
Injection is given carefully in the subcutaneous layer of the skin to ensure optimal survival of the fat cells. Non-surviving fat cells do not turn cancerous but leave behind calcification that may cause confusion during mammography.
Several sessions may be required to achieve the desired outcome.
Qn: What are the various types of breast implants and are they safe?
Ans: Breast implants can be filled with saline or silicone. They are either circular or tear drop (natural profile) in shape.
The most common implant used nowadays contains cohesive silicone gel in tear drop shape. This type of implant simulates the natural appearance of the breast and its content resists extensive leakage even when the implant is ruptured.
Siicone breast implant has good safety record.
Qn: What are the considerations when deciding on breast implants?
Ans: The main point of discussion with your surgeon would include size, type of implant, location of the cut and placement of the implant.
The most common incision is at the breast fold. This incision affords easy access and allows more accurate orientation of tear-drop shaped implant.
Incision in the armpit used to be popular with round implant. Incision around the areolar can affect nipple sensation and limit the size of implant if the areolar is small.
“Most Asian patient with smaller breast volume, the implant is usually placed behind the chest muscle (subpectoral). This placement provides more soft tissue coverage and reduces the incidence of capsular contracture (scarring) around the implant.” Said Dr Ng Siew Weng.
Qn: What is the recovery process after breast implant surgery?
Ans: Breast enhancement is a day surgery procedure. Most patients return to work within a few days and the swelling and bruising would resolve in about two weeks. Self-massage would be taught to hasten the recovery.

What is Facial Rejuvenation? From the simplest to the most elaborate, there is an option for everyone.
1. What typically happens during the ageing process?
The skin becomes thinner with loss of ground substance, collagen, elastic fibres, and occasionally, fat. This is accompanied by uneven accumulation of pigments. This results in deterioration of skin texture and color, wrinkles, skin laxity and loss of volume.
2. How do you formulate a rejuvenation regime?
Every patient requires a personalized regime. I identify the most significant problems and the ones that bother the patient most. I plan the regime based on the patient’s work schedule, tolerance for downtime, pain threshold, risk adversity and financial constraints.
3. What are the simpler rejuvenation techniques?
For improvement of skin texture, pore size, pigmentation and fine wrinkles. I recommend intense pulse light (IPL) or broadband light (BBL). This treatment is well tolerated and has no downtime. Clinical results are usually seen within two to three treatments. Some patients report results as early as one week after treatment.
Botulinum toxin injection is the standard treatment for wrinkles which are worsened by facial animation. It can improve crow’s feet, frown lines and forehead wrinkles and its effect usually lasts for three to four months. Clinical judgment is needed to alleviate wrinkles without causing an unnatural mask-like appearance.
Filler replaces volume to correct deep facial folds and volumize the cheeks for a more youthful appearance. For non-surgical augmentation of the nose, fillers be a temporary alternative. Most fillers are composed of hyaluronic acid, which lasts for about six months. Long-lasting filler materials are associated with side-effects such as infection and granuloma formation.
Botox and fillers complement each other and some patients require both.
Several non-invasive therapies are available to tighten the skin moderately. These include the use of infrared wave therapy (Scition SkinTyte), radiofrequency (Thermage), and focused ultrasound (Ultherapy). Post procedure oedema and erythema commonly resolve within a few days, but clinical results may not be as dramatic as ablative laser procedure.
Microlaser peeol (resurfacing) can further improve superficial skin texture, and the patient can put on make-up about three to five days after the procedure.
4. What are the moderately complex techniques adopted for skin rejuvenation?
Profractional laser is used to achieve further skin tightening. This ablates narrow channels into the skin, and is used to treat atrophic acne scars. The healing process is faster and patients can return to work in a few days. Thread lift provides added skin lifting to the cheeks and temporal area. Several barbed threads made of absorbable material are inserted skin-deep and stablised by suturing to the temporal area. The initial appearance may appear over-corrected but normally improves over a few weeks to months. The effects can last for one to two years.
Fat injection (grafting) has become popular for volume replacement. Fat tissue contains stem cells with rejuvenating properties which are harvested form the abdomen or thighs and processed before begin injected. Post-procedure swelling and bruising last for a few weeks, and the final result is firmer skin with increased volume and improvement in wrinkles.
5. How do you devise a surgical plan for facial rejuvenation?
Ageing affects each part of the face at different speeds and times and this must be understood in the context of the desire for facial rejuvenation. For example, when treating aged droopy eyelids. I normally assess weakness in the lifting (levator) mechanism of the eyelid, excessive eyelid skin, and brow ptosis (drooping). If all these are present, the patient will be advised to undergo levator mechanism correction, removal of redundant eyelid skin an brow or temporal lift.
Another common problem is eyebags, associated with skin and lid laxity. Surgery may involve removal of excessive fat and redundant skin, followed by fat distribution, tightening of the lower lid mechanism and fat injection to improve contour and skin texture. This may be combined with a mini-face lift to achieve a more harmonious appearance.
Some patients requests a lower face lift because of sagging chin and jowl. But since they have moderately severe eyebags, they should consider lower lift surgery with face and neck lift to achieve a synchronized result that looks more natural.
In Singapore, we do not often see the kind of dramatic before and after images that are posed on Korean websites. After a year of training in advanced plastic surgery there, I realised that we have the technology and knowhow here, but patients in Singapore are too conservative to undergo the complex multiple surgical procedures listed above to achieve the quantum leap.
6. Any advise on post rejuvenation procedures?
For optimal outcome, the patient must ensure sun avoidance and regular application of sunscreen especially after laser treatment. After surgical treatment, it is advisable to maintain the result with non-invasive rejuvenation procedures on a regular basis.

5) Implants vs fat grafting vs fillers, what is the difference amongst them three?
Fillers are currently banned as it has resulted in complications.
Both fat grafting and implants are done extensively with implants being more popular as implants allow the patients to achieve whatever breast size they prefer in just one surgery as compared to fat grafting which typically requires more than one surgery as each surgery only allows an increase of ¾ to at most 1 bra cup size.
A patient who wants to achieve a C cup from an A cup will require 2-3 surgeries before achieving their ideal cup size, making the cost of fat grafting much higher than the implant method.
6) Which is better, fat grafting or implants?
There is essentially no difference between the two methods. Some patients choose fat grafting for the sole reason of not wanting any foreign object in their bodies.
As for the result, there is again not much difference between the two methods as gummy bear implants which are natural to the touch, are used, having the same natural touch to that of fat.
7) What are the pros and cons of implants and fat grafting?
Implants allow patients to get any cup size in just one surgery, thus lower in cost. However, the downside to the implant method is that every 15-20 years, the implant has to be removed and exchanged for a new implant to avoid capsular contracture where the body forms a scar around the implant.
For fat grafting, there is a limit to the increase in bra size per surgery and thus requires more than 1 surgery, resulting in an increased cost for fat grafting method. However, the fat that stays after the fat grafting procedure, will stay for life.
8) Which is better, silicone implant or saline implant?
Saline was introduced when silicone implant was banned in the US during the 1980s due to a silicone scare. However, the ban has since been lifted when the scare of silicone implants leading to breast cancer and autoimmune diseases has been found to be untrue.
Since the ban has been lifted, silicone implants are more widely used as it feels and looks more natural than saline implants. One of the common downside of saline implants is that the gurgling sound of the saline moving can be heard when patients are more active.
Thus, 99.9% of the time, silicone is used during a breast implant surgery.
9) Are there any lifestyle habits to change or eliminate before the surgery?
Smoking should be avoided as nicotine constricts the blood vessel. Also, maintain a well balanced diet before the surgery.

And though her lips looked plump after the injections, things went downhill within two months when three hard, wart-like lumps painful to the touch formed on her mouth.
When Miss Grenville returned to the clinic two months later, staff injected her with another fluid they claimed would rectify the problem.
Instead, to her horror, her lips started to swell and turn blue. Later, more yellow, painful lumps formed on top of the initial lumps that already protruded from her mouth.
After researching online, she discovered she may have granulomas, small areas of inflamed tissue, which are a known complication of silicone lip fillers.
By this point, Miss Greville thought she may not have been injected with Juvéderm, and after receiving a refund from Groupon, she contacted another clinic.
The doctor said he believed the clinic had used silicone – a much cheaper substance which is illegal in Thailand.
Miss Greville added: ‘The doctor I spoke to contacted the Juvéderm supplier in Thailand to check whether they have ever had an order from this clinic; they confirmed, they have not.’

After years of living in a body that didn’t belong to her, a transgender woman finally took the plunge and changed her body into one that she had always wanted and dreamt of.
Spending approximately S$105,000 on plastic surgery to achieve her version of aesthetic perfection, Fulvia Pellegrino is by no means ready to stop her aesthetic pursuit.
Till date, the 56 year-old transgender has had more than 150 lip and cheekbone fillers, four breast implants, and two rounds of liposuction of which S$19,000 was spent on a new bum.
But that’s not even the most bizarre thing; Fulvia Pellegrino’s wife, Marisa, is actually fully supportive of her transition.
Both Marisa and Fulvia are married, but Fulvia’s inability to conceal her sexuality eventually caused the 56 year-old Italian to undergo a sex change.
Now, the pair are more like sisters than a married couple.
Although Fulvia is now free and feels comfortable in her own body, she didn’t have it easy in her earlier days. Fulvia was just fifteen when she realised that she was transgender.
However, because of her religious family, Fulvia has no choice but to keep her sexuality under wraps. Her father, a clergy, had three sons and wasn’t about to accept anything different.
She said: “I was living with my family who were very narrow minded and very religious.
“I never manifested my will because it was impossible with my dad.
Fulvia kept her darkest secret with her even when she met her wife, Marisa Altare, 32-years-ago.
While Fulvia seemed macho and a completely normal man on the outside, she secretly frequented gay clubs and cross-dressed in the privacy of her own garage.
“I was feeling trapped in my body. I hid in the garage and put make-up on dressed like a woman.
“I never showed that side of me because I was ashamed of it.” said the Italian.
Masking her internal turmoil, Fulvia often exaggerated her masculinity in public, buying guns and owning expensive cars.
She said: “It is difficult to grow up with a body which is not yours. You try to mask it in every way playing football, drinking beers, buying American 4x4s and go shooting to feel like a man.
But 16 years later, Fulvia finally had enough of her concealment and confessed to Marisa that she wanted a transition.
Although Marisa didn’t take too kindly to the shocking news and even had to visit a psychologist to help her come to terms with the drastic change in her life, she eventually got better and even stood by her husband, supporting her decision.
“Instead you understand it is a mask and so you need to throw it away and become what you are.”
Fulvia began her transition by receiving hormone therapy and even started to look into plastic surgery, to achieve the body she had masked for the past 5 decades.
But even the transitioning wasn’t a smooth process for Fulvia. Even though Marisa had finally accepted her reality, Fulvia’s parents were not even close to accepting their son’s decision.
“My dad asked my wife to sign a paper to send me into a psychiatric institution to make me change my mind.
“It was very hard, when my father passed away they didn’t let us go to the funeral because they were ashamed of us.
“The only people who accepted me were my brothers.”
But despite her family’s turmoil, Fulvia began to pursue her extreme look with breast implants, a nose job and face lift in 2004.
Three more breast augmentations followed by liposuction, numerous fillers in her cheeks and lips and a S$45,000 butt lift.
Although Fulvia runs a restaurant but no longer works, the expenses incurred for her plastic surgery procedures have caused a strain in her finances. Unwilling to give up her aesthetic dream, Fulvia sold her guns, cars and a house in France to pay for her expensive surgeries.
Now the pair struggle on Marisa’s wage as a holistic therapist. And while Marisa is happy to support Fulvia’s quest to achieve her desired look, the neighbours in their town of Peveragno have gone out of their way to exclude the couple.
Fulvia said: “I am now the clown of Peveragno. They point at me, they talk about me, they start inventing stories about me that I am a prostitute.
“My friends have all vanished – they didn’t accept me.
“I couldn’t be without Marisa and do what I do without her. She is always here with me.”
But despite the difficulty she faces Fulvia is desperate to continue her plastic surgery journey.
She said: “I want to look like Allanah Starr, an American transgender porn star.
“Plastic surgery is not a drug but its something that people like me need.
“I am not happy with my face and other parts of my body.
“I won’t stop here, I want to go ahead because it’s not perfect. If I reach perfection maybe I will stop.”
